If you are having an outpatient procedure:
- Report to Day Surgery.
- Bring your financial information and any paperwork you received from your physician.
- You must authorize permits for certain examinations, treatments or surgery (if you are unable to sign the consent form yourself, your next of kin must sign it).
- A responsible adult must be with you when you are discharged because you will not be allowed to drive if you have had any type of anesthesia or sedative.
- For your protection, an identification band will be attached to your wrist and must not be removed during your stay.

Bring only:
- Personal items such as cosmetics, nightgown or pajamas, dressing robe, and slippers
(a kit containing toiletries and shower items is distributed to each patient upon admission).
- A list of medications you are now taking.
If you bring an electrical item with you, please notify your nurse so that a member of our engineering staff may check the item for safety. This is necessary so that your equipment will not interfere with any high-tech electrical equipment used in medical care.
