BETTER COMMUNICATION, BETTER CARE
PATIENT ELECTRONIC MEDICAL RECORD
When your health care team works together, as a coordinated team, you can expect better care and better outcomes. That's why the Nanticoke Physician Network has made a significant investment in an Electronic Health System (EHS). This EHS is a information technology system that allows physicians within the Network to share one Electronic Medical Record (EMR) for each patient.
By using an EMR for a patient, outcomes are better because:
As a part of the EHS, patients have access to their health information and can communicate with their physician offices. Some of things you can do through the Patient Portal are:
Keep Tracking of Your Appointments: View their future appointments, receive appointment reminders by email or text message, request an appointment or view all your previous appointments.
More Convenient Patient Registration: Change demographic or insurance information online to keep your record current.
Medication & Pharmacy Information: Through the portal you can see your current list of medications. You can also record your preferred pharmacy information making filling your prescriptions a little easier.
Account Balance: Displays your current account balance for physicians in the network and information on payment arrangements.
Office Forms: Forms that might need to be completed before coming to a doctor's visit can be downloaded and completed before arrival, speeding up the check-in process during your visit.
Patient Education: Education information you may need can be sent to the online patient portal instead of printing the documents. This allows for easy access to the information you need without the clutter.
The Patient Portal also includes video tutorials to help you while using the portal or to learn about all the ways the portal can help you keep better track of your health information. And, you can set your own preferences and privacy restrictions.
In addition to requesting appointment reminders by email, you can request appointment reminders be sent to you via text messaging. Note that standard text messaging fees apply.
Email and text message reminders are sent 48 hours prior to your upcoming appointment.
You DO NOT have to enroll in the patient portal to receive text message appointment reminders.
For more information about the patient portal or text message appointment reminders, talk with your physician's office or call us at 302-536-5389.
