Nanticoke’s File of Life packets keep your medical information – including allergies, medications and important contacts – all in one safe place. These FREE packets are kept in red plastic pouches with magnetic strips to be stored on the refrigerator so medical personnel can easily find and use them as a reference when responding to a medical emergency.
Nanticoke also has Personal Pocket Medication Cards, mini versions of your File of Life, that can be stored in your pocket, purse or wallet for quick access and convenient storage.
Nanticoke gives out File of Life packets and Personal Pocket Medication Cards at community health events and at the hospital upon request. If you’d like to request a copy of either, please fill out the form at the bottom of this page.
Keep your Forms Up to Date
Keeping your File of Life packet and Personal Pocket Medication Card updated could save your life! Update your files regularly as your medications change and bring your Personal Pocket Medication Card with you to doctor and hospital visits.
