Your case manger is a liaison between you, your physician, ancillary staff, and other outside services and resources. Your case manager will follow your progression throughout your hospital stay, help in the development of your plan of care, and facilitate the continuity of care available to you when you leave the hospital. The case manager is familiar with care options that match your insurance or can help find other resources to provide appropriate care and medical supplies at discharge.

The Case Management department consists of registered nurse case managers, medical social workers, and physician liaisons.
