The Nanticoke Physician Network (NPN) wants to be your home for healthcare. As a medical home, we use a team-based approach to provide total healthcare. Your medical home includes your healthcare team, your family, and most importantly you.
Our goal is to be your partner in healthcare by serving as your medical home. We are committed to serving the community by promoting wellness, advancing preventative care, utilizing evidence-based guidelines, supporting patient self-management, and coordinating the patient’s healthcare across all settings. Utilizing a common Electronic Health Record, we strive to make patient movement within the NPN seamless and transparent.

After office hours, you may request care and advice for non-urgent matters through the Patient Portal. If your matter is of a more urgent nature, call the hospital operator at 302-629-6611 to have the on-call provider paged.

To effectively serve as the patient’s medical home, we request that you provide us with your medical history. We also ask that you provide us with information about any care obtained outside of our practice. This may include:
- Current medications
- Test results
- Visits to other doctors and healthcare providers
- Emergency Department visits
YOUR CARE TEAM
A patient-centered medical home uses a care team, which is a group of people working together to help improve and manage your health.
Our care coordination team is made up of dedicated registered nurses whose clinical backgrounds allow them to efficiently identify needs and coordinate services for patients, making them an integral part of your healthcare team.
Using the care team approach, we are able to provide you with a more personalized approach to comprehensive and coordinated care.
