Nanticoke wants to be your home for healthcare. As a medical home, we use a team-based approach to provide total healthcare. Your medical home includes your healthcare team, your family, and most importantly you.


Our goal is to be your partner in healthcare by serving as your medical home. We are committed to our patients, promoting wellness, advancing preventive care, utilizing evidence-based guidelines, supporting patient self-management, and coordinating the range of the patient’s healthcare across all settings. Utilizing a common Electronic Medical Record, patient movement within the network is seamless and transparent.


Our office hours are 8 AM to 5 PM, Monday through Friday. You may request care and advice during office hours by calling your provider’s office directly. If you need care or advice after office hours, you can contact a member of your care team by calling Nanticoke Memorial Hospital at 302-629-6611302-629-6611 to page the on-call provider.

You also have the ability to contact your care team for non-urgent matters through the Nanticoke Physician Network Patient Portal. If you have not signed up for the patient portal, call us at 302-536-5389302-536-5389.


A patient-centered medical home uses a care team, a group of people working together to help improve and manage your health. The core members of the team always include your primary care provider, the office staff, and you. Other members of the team may include specialists, a care coordinator, and other service providers. Your family or support person is also an important part of the care team.

The Care Coordinator is a member of our team who will identify and help connect you with services that may help you reach your healthcare goals. Your care coordinator can also help with health coaching and education. Using the care team approach with a Care Coordinator will provide you with more personalized, comprehensive and complete care.

When your health care team works together, as a coordinated team, you can expect better care and better outcomes. That’s why the Nanticoke Physician Network has made a significant investment in providing patients an Electronic Medical Record (EMR). Since all the providers in the Nanticoke Physician Network share one EMR, they can easily access and share information about treatment, medications or other other health care needs in real-time. This increased communication improves patient safety, increases collaboration, and leads to more positive outcomes for patients.
